New Role for a Team Leader within the Hospital Discharge Team in Coventry. Full-time, 37 hours a week, £18/hr

Your new company is a health organisation based in the heart of Coventry.

Your new role

You will be working as a Team Leader in the Hospital Discharge Team. The main purpose of your role is to manage, assist and facilitate effective and efficient Hospital Discharge for service users across the pathways. This means you will be working very closely with internal and external key stakeholders including hospital and other internal service users.

Your role entails:

    • Supervision of staff
    • Signing off of assessments and support plans
    • Quality checking of work
    • Agreeing funding requests
    • Provide support and advise
    • Liaise with other management colleagues within the service or with Health partners
    • IT skills an essential as role will involve capturing data and outcomes
    • May need to support Safeguarding decisions or Manual handling clinical decision dependent on qualification

What you'll need to succeed

To be considered for this position, you must either be a Qualified Social Worker or Qualified Occupational Therapist. This role requires an Enhanced DBS Check and preferably a Driving License / car user.

What you'll get in return

This is a full-time role, 37 hours per week from Monday -Sunday (flexibility will be required - this role includes weekend on call rota), paying up to £18.00/hour

This is a flexible hybrid role, where you will be required to work on site and various locations around the Coventry area.
